When the hospital sends your parent home,we help do what’s next.

CareBridge provides non-clinical administrative and logistical support to families navigating complex hospital-to-home and hospital-to-SNF transitions, ensuring nothing falls through the cracks during the most critical weeks of care.

WHAT FAMILIES TELL US THEY FACE

Discharge instructions they couldn’t follow

Missed follow-up appointments

Fragmented communication

The anxiety of “not knowing what we don’t know.”

Hospitals handle the clinical care. Home health handles logistic needs.

CareBridge handles everything in between the paperwork, the logistics, and the follow-through. The coordination gap no one else fills.

Discharge plans made actionable

We translate clinical discharge instructions into a plain-language checklist your family can actually follow with due dates, task owners, and clear next steps.

One organized place for everything

Medical records, insurance paperwork, care summaries, legal documents all stored securely and organized so you can find what you need when you need it.

Follow-through you can count on

Weekly check-ins and 30-day summaries keep you on track across the full month transition through multiple hospital episodes if needed.

The Process

From overwhelmed to organized in four steps.

1. A free consultation call

We spend 30 minutes understanding your family's situation your parent's diagnosis, where they're being discharged to, and what feels most unmanageable right now. There's no obligation, and we'll be honest if we're not the right fit.

2. Essentials Setup

We conduct a thorough needs assessment, build your secure document system, and convert existing care plans and discharge summaries into a structured task checklist so you always know what needs to happen and when.

3. Ongoing Navigation Support

Weekly check-ins and a 30-day family summary keep your task list current, your documents organized, and your follow-up on track throughout days 3-30 engagement and through repeat hospitalizations, if they occur.

4. Warm Handoff When Ready

When the acute transition period is behind you, we close out with a complete summary of records, contacts, and completed tasks, and a clear list of resources if you need support down the road. The family portal remains active, with a monthly subscription.

How pricing works

Three phases. You only pay for what you need.

Everything is billed at a flat rate of $135/hour. Before each phase begins, you receive a quoted estimate and a clear hour range and dollar range, so you always know the cost before any work starts.

Meet the Founder

Built by someone who lived it.

I didn't start CareBridge because I saw a market opportunity. I started it because I sat in a hospital discharge meeting with my own family and walked out holding a folder of instructions I couldn't make sense of and a parent whose care, for the next thirty days, depended on us figuring it out.

The doctor had done her job. The nurses had done theirs. But nobody was responsible for what happened next for translating a twelve-page discharge summary into a plan a working adult child could actually follow. For knowing which appointment needed to happen first, or which forms needed to go where, or what to watch for at home that would signal we were heading back to the ER.

That gap nearly cost us. I spent weeks piecing things together through phone calls, late nights on the internet, and a lot of educated guessing. We got through it but I kept thinking about the families who don't. Families without the time, the resources, or the confidence to push back and ask the right questions.

CareBridge is the service I wish had existed. It doesn't replace doctors or nurses or home health aides. It fills the space between clinical care and family execution the logistics, the paperwork, the follow-through so that families of CHF and COPD patients in northern New Jersey can focus on being present, not overwhelmed.
